Annual 4-H Group Activity Report - County Educator Summary

Dear County Extension 4-H Educator, Please compile all data from the "Annual Report on 4-H Group Activity" from 4-H groups in your county. You may print this blank form and use it as a paper copy to aide in preparing the final report. When all data is tabulated and summarized, enter it on-line using this form.
