fbx_FormBuilder by UNHCE is a web-based application that enables end users to create and publish simple on-line forms and surveys with little or no intervention by IT support staff. This application, licensed under the CC-GNU GPL, may be freely downloaded and redistributed in accordance with the license.
fbx_Formbuilder requires a web server running ColdFusion (Version 7.01 or later, it may run on 6.1, but hasn't been tested), ability to run MS Access, MS SQL, or MySQL datasources, and appropriate permissions to be able to write html files to the server in at least one directory.

The web provides an excellent means of gathering feedback and responses from readers at very little cost. However, creating the instruments to gather the feedback typically falls on the shoulders of IT staff. This can create a barrier to the creation and use of such instruments, regardless of their utility.
fbx_Formbuilder allows registered end users to create their own forms, publish them, and receive the responses, without requiring a knowledge of HTML or server technology. Through a form-based user interface, users create new forms; add, edit, delete, and reorder questions; publish the form to the web server; and view or download the results. They may also choose to have the results emailed to them as they come in.
fbx_Formbuilder can be used to create surveys, feedback and comments forms, event registration forms, and more. fbx_Formbuilder supports several question types, including single-line free text, multiple-line free text, select only one from a list, select one or many from a list, matrix type (typically applying a scale to several consecutive items), and comments or instructions. The form creator can also require that individual questions be answered prior to form submission.

Certain functions have been disabled to maintain the integrity of the demonstration site (e.g., can't receive email responses, can't share forms with other users, can't modify user profile, and can't delete forms.) Please don't use the demonstration site to try to collect useful data, as it may be periodically purged of existing forms and results.
